How to Pay for Our Services
Please familiarize yourself with your insurance plan
and benefits covered. Always have your insurance identification
card available at the time of your appointment. You
are obligated to pay for any co-pay or deductible as
determined by your insurance company. If you have Medicare,
we will submit the claim for you. We are participating
providers with Medicare, and are paid directly by Medicare.
You are obligated to pay the Medicare deductible. If
you have an insurance that is secondary to Medicare,
please give this information to our front desk staff.
We will be happy to submit the claim for you after Medicare
has paid us. If you have an insurance that is an HMO,
we will submit the claim for you. PLEASE NOTE THAT THE
COPAYMENT AMOUNT LISTED ON YOUR INSURANCE CARD IS PAYABLE
AT THE TIME OF SERVICE.
If you have an insurance that is not one with which
we participate, we can submit the claim for you if you
desire; in this case, our policy is that you pay at
the time of service. If you have Medi-cal, please present
your medi-cal card upon your arrival in order for us
to verify eligibility. You will not be seen for a non-emergency
appointment if it is determined that you are not eligible
that month. Please be advised that it is your responsibility
to verify with your insurance carrier if our office
is listed as a participating provider, and to obtain
any necessary authorizations. You are ultimately responsible
for any medical services received at our office regardless
of the decision involving reimbursement made by your
